Build an Onboarding Form
Design a branded intake form with mapped fields, document uploads, e-signatures and conditional questions, then publish it.
Module: Onboarding.
Overview
An onboarding form template is the intake form your vendors fill out. Each field maps to a field on the vendor record, so approved answers land in the right place automatically. 🧩
Create the form 🛠️
- Go to Settings → Onboarding → Onboarding Forms (or Onboarding → Form Templates) and click Create Form, or open an existing template.
- On the Edit tab, click Add Element to build the form:
- Text Block: instructions or a welcome message.
- Text Input, Email, Address and Single Select: answers your vendor types or picks.
- File Upload: documents such as a W-9, certificate of insurance, EMR or OSHA records, each tagged with a document type.
- E-Signature: the vendor draws or types a signature.
- Map each element to a vendor-record field (the builder shows the mapping, e.g. Tax ID or Auto-mapped for addresses). Elements marked Not stored are kept on the submission only.
- Make elements conditional to show them only when they apply, e.g. ask for a bond certificate only when Type of Work is Exterior.
- Use Preview to see the form as your vendor will.
Branding 🎨
On the Branding tab, add your logo, Import from Website to extract your colors, and choose a primary color, background style and button shape.
Settings ⚙️
- Template type: Normal (collects vendor information and documents), Pre-Qualification (unlocks the vendor Pre-Qualification dashboard) or E-Signature (a legally executed document with ESIGN consent, countersigning and a sealed signed PDF).
- Thank-you screen: the heading and message vendors see after submitting.
- Route approvals to specific approvers: off, anyone who can review onboarding approves; on, submissions go to the approvers you choose.
- Delivery: Per-Vendor Link (tied to an existing vendor) and/or Public Link (creates a new vendor record on submit).
Publish ✅
Click Save Draft while you work and Publish (or Republish after edits) to make the latest version live. The header shows the version number and how many elements are mapped.
